Abstract
A Failure Reporting Form has been developed by the IEEE Switchgear Committee to improve circuit breakers reliability through uniform reporting of field failures. The use of the Failure Reporting Form is recommended to standardize reporting of field failures and to use these experiences to improve the power circuit breakers reliability.
